Final expense insurance with a cancer history

Yes, and timing matters most. Carriers look back 2 to 4 years from your last diagnosis or treatment. Mutual of Omaha declines cancer within 2 years and offers its graded plan from 2 to 4 years. Foresters places cancer within 3 years in its graded tier. Current, metastatic or recurrent cancer usually leaves guaranteed issue as the option.

Why the date of your last treatment matters

Cancer questions on final expense applications are built around time. They ask whether you have been diagnosed, treated or advised to get treatment within a set number of years. The clock usually runs from the most recent of those events, not the original diagnosis.

Ask how a carrier treats ongoing medicines taken after surgery, chemotherapy or radiation. Carriers can define treatment differently, and the answer can move you between plans.

Many people face this question. The National Cancer Institute reports 18.6 million cancer survivors in the US as of 2025, and 79% are age 60 or older.

Common lookback periods: 2, 3 and 4 years

Mutual of Omaha's Living Promise uses two lookbacks. Cancer within the past 2 years makes you ineligible. Cancer within the past 4 years points to the graded plan, and after 4 years you may qualify for the level plan.

Foresters PlanRight uses a 3-year lookback. Cancer diagnosed or treated in that window places you in its Basic tier, which has a graded death benefit. After 3 years, cancer is no longer asked about unless it is current or has recurred.

Royal Neighbors of America asks about cancer, melanoma or brain tumors in the last 10 years. Its form says a yes answer does not disqualify you from all offers. Our waiting periods guide explains what graded means for your family.

Skin cancer exceptions

Not every skin cancer counts. Mutual of Omaha excludes basal cell and squamous cell skin cancer from its cancer questions. Foresters and Royal Neighbors exclude basal cell skin cancer.

If you are unsure which type of skin cancer you had, check your pathology report or ask your doctor before you answer.

Melanoma is treated as a full cancer. Mutual of Omaha and Royal Neighbors name it specifically in their questions.

Metastatic, recurrent or current cancer

Cancer that has spread or come back is the hardest case. Mutual of Omaha excludes anyone ever diagnosed with metastatic cancer or a recurrence of the same type. Foresters excludes anyone who currently has cancer or has had more than one occurrence.

A terminal diagnosis is also a knockout. Both carriers decline an illness expected to cause death within 12 months.

Brain tumors can have their own question. Foresters places a brain tumor in the past 2 years, or one still being monitored, in its graded tier.

In these cases, guaranteed issue is usually the remaining choice. Plans from Mutual of Omaha, Corebridge and Gerber Life ask no health questions. If death from illness occurs in the first 2 years, they pay back premiums plus interest instead of the full benefit.

What to have ready

Write down the type of cancer, the date of diagnosis, and the date of your last surgery, chemotherapy or radiation. Those dates decide which lookback applies.

Note any recurrence or spread, even if it was treated successfully. Mutual of Omaha and Foresters both ask about recurrence or more than one occurrence.

List current medicines too. If you still take a cancer-related medicine, ask the carrier whether it counts as treatment before you answer.

Graded versus guaranteed issue for cancer survivors

If you are past a carrier's knockout window, compare its graded plan with guaranteed issue before you choose. Graded plans still ask health questions, so they suit people who clear the knockouts.

Early-death payouts differ by plan, so ask for the exact schedule. Our waiting periods guide compares the common designs.

Guaranteed issue remains the fallback when cancer is current, has spread or has come back. It can also bridge the gap while you wait for a lookback date to pass.

Pending tests and how to apply

Wait for pending results before you apply. Mutual of Omaha and Foresters both exclude applicants with tests or treatment advised in the past 12 months that are not yet done.

Answer every question truthfully. Policies are usually contestable for their first two years. In New York, for example, the law lets an insurer contest a claim in that window if it proves a material misrepresentation.

If you are close to a lookback date, ask an agent whether waiting a few months could move you to a better plan. You can compare quotes or see our guides for people over 60 and over 70. Availability varies by state and carrier.

How carriers treat cancer

Based on each carrier's application questions or product page, linked below. Forms change and vary by state, and the current application always controls. Final approval rests with the insurer.

Questions people ask

Can I get final expense insurance if I had cancer?

Yes. Once you pass a carrier’s lookback period, often 2 to 4 years from your last treatment, you may qualify for a level or graded plan.

Can I get final expense insurance while I have cancer?

Usually only guaranteed issue. Carriers such as Mutual of Omaha and Foresters exclude current or recent cancer from their underwritten plans.

When does the cancer lookback period start?

Most questions ask about diagnosis or treatment within a set number of years. The clock usually runs from your most recent treatment.

Does basal cell skin cancer count?

Not on the Mutual of Omaha, Foresters or Royal Neighbors forms we reviewed. Mutual of Omaha also excludes squamous cell skin cancer.

What if my cancer came back?

A recurrence is usually treated strictly. Mutual of Omaha excludes recurrent cancer of the same type, and Foresters excludes more than one occurrence.

Should I wait to apply until I pass a lookback date?

Sometimes it helps, but delay also raises your age-based price and leaves you uncovered. Ask an agent to compare both options.

Do internal cancers like breast or prostate cancer count?

Yes. Only certain skin cancers are excluded from the questions we reviewed, so other cancers must be reported within each lookback.

Can I get final expense insurance if my cancer is in remission?

Yes, once you pass the carrier's lookback. Mutual of Omaha looks back 2 and 4 years, and Foresters 3 years, from diagnosis or treatment.
